Our Payment Policy
Devine Intervention Physical Therapy is not affiliated with any insurance company. We do not accept any private or government-sponsored health insurance, including Worker’s Compensation and Medicare.
If Medicare is your primary insurance, we may not be able to see you – even if you want to pay cash.
We may be able to see Medicare patients for non-covered services such as craniosacral therapy, visceral manipulation, or maintenance.
We do not accept clients who are involved in an active injury-related lawsuit.
We accept cash, personal checks, and Venmo only. Payment is due at the time of service.
If you plan to apply for reimbursement from your insurance company, you should find out if they will require you to obtain a physician’s referral before your first visit.
To help our clients with reimbursement, we will provide a physical therapy billing statement which can be filed with your insurance company.
Treatment notes can be provided on request, for a fee.

Devine Intervention Physical Therapy will continue our fee schedule as is. Treatment frequency and duration will be determined through both patient and therapist input with regard to each individual’s goals. Please let us know if you have any questions regarding fees for service.
